Levos Care Team

Based on the patient’s status and goals of care, the care team may also include:

Physicians

Practitioners provide house calls as well as specialists in respiratory, occupational and physical therapy are heavily involved in Levos Care. They provide house calls to address symptoms or other concerns that patients and families may have.

ARNPs & RNs

Make home visits to check in on you and your family and talk with your physicians about how you are doing and any changes needed to your care plan.

Social Workers

Help the patient and family explore coping with an advanced illness and the changes it creates within the family.

Chaplains

Discuss issues and feelings related to spiritual and religious beliefs with the patient and family.

Volunteers

Offer companionship, emotional support, running errands, transportation and respite for family members who need time outside the home.

Home Health Aide

Provide personal care services such as bathing, hair care, dental care and other elements of daily living.
